Title: Completed 6cm femur LL with dr mangal parihar using precice 2.2 (8.5mm) AMA

My original target was 8 cm but I got early consolidation at 6cm. Will go for rebreak in the next 6-9 months. AMA

How come you didn’t increase the lengthening rate to avoid early consolidation?

Is the hospital still very ugly??? Or they improved ir?? Is LON safe there?

It's not ugly at all. It's clean and looks fine. I don't think LON is safe anywhere. I wouldn't recommend LON.

How come you didn’t increase the lengthening rate to avoid early consolidation?

I was doing 0.9 mm per day initially then doc increased it to 1.2mm per day but I still got it even after 1.2 mm per day. I even stopped taking calcium supplements but couldn't stop my bones from fusing together.  PT is mediocre at best. I didn't do much PT throughout the process. Overall exp with doc is pretty good they know their stuff and took all the precautions during and after the surgery.

what’s was the weight bearing protocol and are you weight bearing now? I remember doctor said that he will be more aggressive with weight bearing as he does more precise patients.


Now that you are 6cm taller how does it feel and what’s your starting height if I may ask?


How come you will re-break after 6-9 months and not now?

1) why rebreak? just do tibias and get good proportion
2) how did u travel outside of hotel to hospital? and did u have any other medical needs to go to a doctor?
3) what was the protocl taken to prevent blood clotting

1. I don't have budget to buy another precice for tibia and I won't do external.
2. I am living in a rented flat with 2 flatmates/friends. I use a wheelchair taxi service called mobicab.
3. They put you on dvt pump after the surgery and prescribe blood thinners for 30 days but you can continue to take them for 3 months if you want. I didn't take any blood thinners at all.

I remember discussing all the risks before the surgery. They try to fix non-union by some medicines first and then if it doesn't work then they do bone grafting.

I am walking with a walker and putting full weight on my legs while standing. Hopefully, I can get rid of the walker in a few weeks. I don't feel much different tbh that's why I'll go for rebreak. My initial height was 164cm.

I used a caretaker service. It's not possible without a caretaker or family member.

Nope, just rebreak and more lengthening in femur for now. No immediate plans for tibia lengthening.

1. If you are talking about knee bending then it's like 110 - 120 degrees.
2. 3-4 times a week (not much). My hamstrings are very tight.
3. Yes
4. Having a positive mental attitude and a good support system is very important.

1. I was mainly scared of dvt and fat embolism. The risks are almost the same as getting a femur fracture so I decided to take that risk.
2. I was very clear about getting it done so the only hard part was to convince my parents.

4. No, I am still short. I need to be at least 5'8 to cure my height dysphoria.That's why I'll go for rebreak.

Doc said he'll not rebreak the regenerate part and I think it'll be better to do stretching so that I can go full 8cm and even more if possible. I don't want to rebreak and stop again because of some muscle or nerve issue so better to take some time to stretch.

I had some kind of allergic reaction to both aspirin (aspirin-induced asthma) and Xarelto (stomach issues) so I was using a DVT pump instead.

how can you go even more if the nail cannot extend beyond 8cm? you wll have to change the nail to a new one.

its quite serious to not take blood thinners. i hope you figure something out before your next surgery if u get dvt the treatment is mainly blood thinners.better to find a medication that works for u. why not take injections that will prevent stomach issues?

next time, please see about injections  as blood thinners. they can help avoid stomach issues. dvt is a serious risk with this type of procedure. i think if you had informed your doctor about your problems with tablets he wouldve advised it himself

Why not LON? Is the only option I have becuase of money, externals are too uncomfortable and too long process

LON is the same as externals for the lengthening phase which is very uncomfortable. I was only able to sleep on my stomach and side because of precice this wouldn't have been possible with externals. Also, LON has a risk of infection so you have to be very strict with cleaning and taking care of the wounds.

You can't really do anything. If you increase the speed of distraction then you'll start developing nerve issues. I was doing 1.2mm per day and still got early consolidation.

They are very obvious doc's don't have to do anything special to monitor those. You'll start developing weird sensations and numbness. He mainly monitors the tightness and Xray.

I can walk with crutches and for very short distances, I can walk unassisted.

The walk is not 100% normal but people don't notice anything wrong. I can jump, run and do squats.

It's hard to put it on a scale. It was painful but bearable. After paracetamol, it was like 1-2 maybe 3 max and without paracetamol, it was 5-6. I didn't take any other pain medicine.

I am not sure what you mean by respecting your privacy. They don't sell your data or anything. It's a hospital or not facebook.

They don't advertize at all. They don't accept cosmetic patient easily. Cosmetic LL is not his main practice and he rarely do cosmetic LL (like 3,4 cases per year) so ads are out of the question.

what about that company (not nuvasive something else) which will buy nails for your behalf? is it enough to just transfer them the money and they will buy it or should you share any private informations to them? Did this concern you?

No, I didn't talk to any past patients. I know 2 guys who started 2 weeks after me.

Any advice for finding long stay in mumbai near the clinic?

the options are

* air bnb
* service apartment (costly but you will get care from staff)
* hotel room (no kitchen so have to order food)

any learning from your experience you can share pls?

Near the clinic is very expensive you should get one in navi mumbai or thane

1. Yes, I took their 6 hours 90 km plan.
2. It's fully accessible from the main gate to the consultation room. Most people who go there are recovering from some kinda ortho condition like hip replacement or fracture.

I didn't donate blood and I didn't need any transfusion. Most people don't need any transfusion for this surgery. If it becomes necessary then they'll get it for you it's not a big deal. The place is surrounded by multiple big hospitals and blood banks.

4 to 5 cm in tibia with precice. I'll go with dr parihar again because other options don't make much sense. I've done it once so won't be difficult to do it in the same env again.

nope. I know people who got blood transfusion because of dengue and accident in India so it's very common to get blood and people generally don't talk about the NAT testing issue you mentioned. I think there's not much awareness about this issue.

No one really noticed tbh. Most of my friends are tall so I am still shorter than them but I def feel better.

They don't advertize at all. They don't accept cosmetic patient easily. Cosmetic LL is not his main practice and he rarely do cosmetic LL (like 3,4 cases per year) so ads are out of the question.

A. I think he avoids young people who wants to hide it from their family and people who have unrealistic expectations and doesn't understand the risks.
2. And how did you manage that he chooses you?
A. I think he'll do it for anyone as long as you understand the risks, have realistic expectations and some kind of support from family etc.
3. How much did the surgery cost and how much did you pay for all the time you stayed there?
A. around 42k usd for nail and surgery and per month cost was around 1k$ per month (including rent, internet, food, caretaker, and other misc stuff). My rent was very cheap because I stayed with my friends otherwise it'll be around 1k just for rent) so you can assume 1.5-2k in total per month.

when travel thru mumbai city to get surgery did you feel some what scared because of how much chaos and dirty it is (like dharvi and surrounding areas) in general as you travel thru?

I know this does not mean anything about the hospital but on the journey to the hospital when you see the surrounding did you suddenly feel extra scared thinking what I am doing?

The route from my place to the hospital was pretty clean and didn't have any problems other than traffic. I didn't cross dharvi. I saw a lot of skylines and greenery on the way and the under-construction bridges for the metro.

I did research for this

hepatatis in most cases it goes away on its own so its not chronic in most cases

Getting hiv chances are very very low so I think it might not be big problem really

When I was walking no one cared but when i was on wheelchair people did ask what happened and I just said bike accident and they left me alone.

did you feel embaressed or shy to go there atleast in the first 1-2 time? bcoz its not a cosmetic clinic like the los Angeles mahboubian clinic, so they dont see people for cosmetic surgery frequently

are u sure the dr will accept you as patient again for surgery? bcoz youve done it once already and he might ask "why do you wanna do it again now, are you having depression?"

1. Of course, I'll need another set of precice tibai for 2nd surgery.

2. You don't have to worry about this. The agent will take care of this. It's a simple process.

The first 2 weeks are hard. I know what to expect though. The biggest concern is the stomach and constipation.

Will start taking fiber supplement 2 times from day one. Wil avoid nermax (also called lyrica) I had bad withdrawal syndrome.

The walk is not 100% normal but people don't notice anything wrong. I can jump, run and do squats.

2.Its impossible to walk during the lengthening phase with precice 2.2 right ? unless you weigh like 50kg (I'm kinda confused regarding this.People on the forum keep saying you are wheelchair bound for 6 months if you do precice 2.2 blah blah , but according to Dr paley , precice 2.2 patients can walk around using crutches/walker,while stryde patients can walk without anything within a few weeks of surgery . What's going on ? )

3.You said you didn't have money to do tibias , but now you're going for precice tibias ?

2. It's very risky to walk. I didn't walk at all and didn't put any weight on my legs until I got early consolidation.

Just had a zoom call one day before the surgery with my parents. He just wants to make sure you have proper emotional support during the process.
