Title: Groplate
Post by: precice strider on September 10, 2020, 08:13:17 PM
Does anyone know much about it? It's on Dr. Paley's website and on an Instagram announcement.

Perhaps it's like an artificial growth plate? Like having your growth plates back?

Might there be a spine option? If so people like me with long legs would be able to lengthen more.

from one of the sh1tty interviews he did
https://www.youtube.com/watch?v=-cuqlESgJg8
it seems to be an expanding plate which mean no need for intermedullary nailing
which could be a dream come true for tibs

sorry for highjacking your thread

i found this quote of paleys limblength.org site via goog

Together with Nuvasive, Dr. Paley developed a stronger nail made of stainless steel called the STRYDE nail. Dr. Paley implanted the first STRYDE nail in May 2018. This is the ideal implant for stature lengthening. Dr. Paley also developed the newest device which is the GroPlate which is the first FDA approved fully implantable lengthening plate instead of a nail.

The Precice Plate represents a first-of-its-kind limb lengthening solution for pediatric patients that allows for internal lengthening while avoiding potential damage to their growth plates. Previously, this was only possible with bulky, less comfortable external fixation devices,” said Dr. Dror Paley, director at the Paley Orthopedic and Spine Institute at St. Mary’s Medical Center in West Palm Beach, Florida. “Now, with the Precice Plate, I can finally offer these patients a minimally invasive technique to lengthen their bones for the correction of limb length discrepancy.”

The Precice plating system is the latest advancement in the Precice portfolio of non-invasively adjustable implants from NSO. Leveraging a locking plate technology, the Precice Plate is mounted on the outside of the patient’s bone to avoid potential damage to the growth plate of this young patient population. Once implanted, the patient or caregiver uses a programmable external remote controller that magnetically couples with the Precice Plate through the skin to perform precision micro adjustments. Over the course of several months and from the comfort of their own home, the patient follows a custom lengthening schedule to gradually distract the Precice Plate, stretching newly forming bone to a targeted length. Once the desired bone length is achieved, the plate is removed.

sounds like it may not be fully weight bearing tho

The Precice Plate was designed by NSO in collaboration with a team of world-leading limb lengthening reconstruction experts who will treat patients on a limited basis this year. NSO plans full commercial launch of the Precice Plate in the US and Europe in early 2021.

may have to put LL plans on hold until this is available

Looks like the typical plating they currently use for fixing bowlegs or x legs

That thing looks very fragile. Probably only for kids, as intended in the description. The issue with intermedullary nails with growing kids with growth plates is that it disturbs the ephyphiseal plate, which are both on top AND bottom of the long bones. so pretty external fixators is the only way for GROWING kids to fix a discrepancy. Again, it looks very fragile. Watch a 150lb man stand for just a second and crack that thing

I don't think this is useful for Femurs-

But, for Tibias, this prevents reaming of the bone canal thru lifting the patella  which removes the chances of the infamous "permanent knee pain"

However, I don't think doctors can trust patients won't try to stand on this. This growplate will probably have significantly less weight bearing capabilities than even the PRECICE 2.2 nail. (It's a damn plate, with weight shifting onto one side not a nail in the middle) Stand on it and walk instinctively by accident a few meters as a 140 lb man (instead of like a 70lb child it was meant to be for), and you'll probably snap this thing in half.

I don't think this will be permitted for cosmetic purposes and especially not in adults.

Tldr :
It's meant for kids, like 65lb 8 year olds lol
s fragile as fucc, would probably break if a grown man walked on it for even a little bit. Definitely not weightbearing and probably less bearing than the PRECICE 2.2
But it does prevent reaming of the canal, resulting in less chance of the permanent knee pain for tibias

However doctor will likely not use this for cosmetic purposes. I don't even think u can use crutches with this. It's a plate!!

just like the stryde nails look just any other rod

yeah probs cant weight bear, but i would defs prefer to completly elimate the chances of knee pain down the line and be immoble for 2 months

Usually 1.5 to 2 months per cm for pure externals/no nail until consolidation for tibias. So 5cm lengthening is about 7.5 months in wheelchair in best case scenario

the announcement/ full details should come out tomorrow so we'll see. normal interlocking plates allow for weight bearing so i doubt youll be wheelchair bound for that long

if anything once distraction is finished, you could probsput another medial plate in and be g2g until its time to remove hardware

Guys, I don’t want to get your hopes up, But this implant was designed in our center by John Herzenberg and Shawn Standard, Along with Paley and Mark Dahl.

this is targeted for Kids with open growth plates that cannot be violated by a nail.

It is not intended to be weight bearing, and for now will only allow 4cm of lengthening for troubleshooting purposes.

Stryde is still the best option for CLL.

Is this device much less traumatic than Stryde?

no at all.  Quite the contrary, for the moment until surgical technique improves, it will require bigger surgical exposure.

It is designed for pediatric (kids) patients whose growth plates cannot be violated.
