So if reaming the canal creates the risk of fat embolism I'm assuming it has something to do with the bone debris floating around in the canal, and since the debris is never removed...is there forever a risk of fat embolism?

I think it's a blood flow issue not a bone fragment issue.  You don't want all the bones in your legs full of metal instead of marrow.  Your bone marrow is where your blood cells are created.
