Just want to let you know I really appreciate your diary so much. I am very likely going to Dr. Barinov in the summer of 2016 and this diary is invaluable. Thanks so much for taking the time :)

Can I ask what rate did you lengthen?

it's likely I will be doing surgery with barinov, sticking to 5cm only so i guess ilazarov would be prefered over LON or LATN?

is ketarol taken orally or is it some special injection, and if it is pills can you not just have them ready to take by your bedside along with a glass of water.

I read Kamui's diary and it was said the pain was very bad on the first night, is that because he did not have access to ketarol?

I am also confused about this general anesthesia that you are offered during surgery, people were saying if you take this you wil get a bad headache for days later on, but then it was said it might be somethign elese that causes the headache, if you can give some advice that would help a lot.

are there any patients that have had their devices removed in their own country, and if so does barinov expect us to send the device back to him.

for going home for consolidation, how I will get through airport the device on, should i bring some very baggy joggers, maybe I should bring my own wheelchair to russia, better yet maybe i should go to russia in a wheelchair so i know how to access the planes as a disabled person.

can I check that during the consolodation phase you can go up about 15 stair steps by sitting down on the first step and dragging yourself up each stair, I kindof need to do this to get to the 2nd floor of my house.

also during consolodation, what would be my prefered stance, should i be sitting in wheelchair most of the time, or should i be lying in bed most of the time.

also about wifi, i understand the hospital has no wifi, but does the clinic have wifi, I will only use the internet to change my flight times towards the end of the lengthening phase.

this forum wont let me edit my post, even before another person has posted, so sorry for the triple post.

Another important question, are you charged in EUR or in Russian Ruble, as it seems the exchange rate is better groing from 605,000 Russian Ruble to GBP, than going from the quoted 12,500 EUR to GBP

can I ask, on the first night of extreme pain, can the doctor not simply knock you out? why most you endure it?

Is the rod inserted immidiately after lengthening is done? How soon after rod insertion can you walk? And is it walking unassisted or assisted recommended?

Hello and thanks for sharing your experience!

First time I see Barinov's work!  Can you ask him why he puts the rings so far apart and why he uses only 2?  Doesn't it affect the stability and strength?  Especially since they are connected with long rods that have a very short union at the centre?

I can see TSF using only 2 rings as they have diagonals rods like in construction, making them extremely sturdy and stable, even under lateral force but for Ilizarov I thought it was better to put 3 rings?

Can I ask what rate did you lengthen?1mm/day

it's likely I will be doing surgery with barinov, sticking to 5cm only so i guess ilazarov would be prefered over LON or LATN?
lon and latn are ilizarov.
all patients in 2014-2015 with ilizarov LON had major problems, contracture, nervepain, loose screws. avoid LON.

is ketarol taken orally? yes, if you buy yourself from the 1st floor pharmacy in hospital

or is it some special injection?yes, free injections every night or on pain is mentioned in diary.

I read Kamui's diary and it was said the pain was very bad on the first night, is that because he did not have access to ketarol? I do not know. take all the injections they give you the first weeks. don´t wake up in pain. it sucks.

I am also confused about this general anesthesia? you are given a choice, spinal anestesia, marcaine, and stay awake.
or general anestesia and sleep.
i wanted to be awake to make sure i got painkiller injections in time.
waking up in pain was not part of my acceptable risks.

people were saying if you take this you wil get a bad headache for days later on?
yes, i have heard that most patients get bad headache for days.
i did not. just a heavy head. no pain.

but then it was said it might be somethign elese that causes the headache, if you can give some advice that would help a lot? keep head horizontal.

are there any patients that have had their devices removed in their own country, and if so does barinov expect us to send the device back to him? i dont know. seems risky and complicated.

for going home for consolidation, how I will get through airport the device on, should i bring some very baggy joggers?yes

maybe I should bring my own wheelchair to russia? ...

can I check that during the consolodation phase you can go up about 15 stair steps by sitting down on the first step and dragging yourself up each stair, I kindof need to do this to get to the 2nd floor of my house.? sounds difficult. not impossible. more risk of breakig pins. stairs are very very cumbersome with frames.

also during consolodation, what would be my prefered stance, should i be sitting in wheelchair most of the time? no. you should walk mostly to consolidate bones. then lay down and elevate legs. minimal sitting in anything.

also about wifi, i understand the hospital has no wifi? no wifi
but does the clinic have wifi?yes 250kbit.just get own internet with usbdongle is cheap for one month 30gb and unlimited gb during nighttime costs 700 rubles.

Another important question, are you charged in EUR or in Russian Ruble, as it seems the exchange rate is better groing from 605,000 Russian Ruble to GBP, than going from the quoted 12,500 EUR to GBP? pay by bankwire for exact amount. otherwise they will calculate the exchange rate and usually it is not favorable.

can I ask, on the first night of extreme pain, can the doctor not simply knock you out? why most you endure it? i did not have extreme pain, it went to 5 of 10 for some minutes. nurse gave me ketarol injection. pain went away first nights like that and completely away in couple of weeks.

Is the rod inserted immidiately after lengthening is done? yes. doc expects good regeneration before surgery of nails so walk alot.

Can you ask him why he puts the rings so far apart and why he uses only 2? he has habit of maximising the distance with the same location of pins regardless of leg lengths. patients can probably request more comfortable location of rings for bendability of knees etc.

Doesn't it affect the stability and strength? have not seen anybody with problems, although some patients have had bending of the ridged rod inside the enveloping rod. kindof major 3-6 degrees bending, but apparently no problems....

Especially since they are connected with long rods that have a very short union at the centre? there are longer rods that can  be ordered. they change your rods.

I can see TSF using only 2 rings as they have diagonals rods like in construction, making them extremely sturdy and stable, even under lateral force but for Ilizarov I thought it was better to put 3 rings? don´t know

more lymphorrhea from left ankle

98
broke a pin on the left while walking.
right foot in front and extending left calf.
sproing!
no pain at all but some panic.
pin was creaking with that exact movement for 10 minutes beforehand and for a couple of days...
external breakage, luckily.
not inside the leg.

same situation on the right leg.
it is also creaking.
not broken yet.

had to wait 4 hours
saw the doc but he had a surgery at the hospital first so...wait.

fixing it was...painfulish, or well at most 20% of 100
because it was like a creepy pain.
never felt anything like it.
surprising really.
kind of like at dentist when they pull out a tooth.
but it is your legbone.
the pain is biting but also numbish and it reaches to your skeleton.
not nice.
not really painful.
but very unpleasant and creepy.

doctor pushes the longer end in. creepy pain.
then refixates the pin which bends the pin towards the circle.
more creepy pain.
not nice.
very quick, 5 minute procedure in total.
but not nice.
no more walking today

99...

100...

101
walking much more carefully
stretching more

I saw your desired length was 10cm how much did you achieve when you were done? Does Dr. Barinov offer standard Ilizarov without nail insertion? I only want to do up to 6cm at most and dont wish to do LON or LATN.

Hi,

Thanks a lot for your informative diary!

I've been talking to Irene and it seems that Volgograd only offers LON or LATN for foreign patients now (as opposed to external only) due to immigration services imposing restrictions. I asked about permanent knee pain from LON or LATN. She said that due to Dr. Barinov's regime no patients have experienced knee pain so far. As you stated to avoid LON due to all patients in 2014 to now experiencing major problems, I'm assuming this isn't true? Also, is this the case with LATN as well?

Also, is this the case with LATN as well?

right pin, dont know which, is creaking slightly when stretching the calves, so i walk differently, with crutches behind legs, because it is always when bending forward with crutches in front that a creaky pin breaks from extending the calf.
the movement automatically maximises the stretching of your calves and if your calves are stiff, it strains the pins.

so if there is creaking, stop walking and stretch.

can the device be removed at the end of the consolodation by yourself or does it need to be an operation of sorts and a return to russia assumiing you went home for consolodation.

I am just trying to compare mitkovic device with this device, i know with mitkovic device you can remove it on your own.

Removing it on your own and risking to get a bone infection because the pinsites were not cleaned properly. Great idea!
